| To claim your cashback, Simply follow these steps |

- Purchase any product from the qualifying product list below between 1st May - 31st May 2008. All claim forms and supporting documentation must be received within 30 days of the date of purchase. See terms and conditions for full details. Complete the claim form. Attach invoice to the claim form, with product(s) and purchase date(s) circled.
